Terms of Service

These Terms of Service (“Terms”) govern your use of Credentialed - the credentialed.health website, the Credentialed web platform, and the Credentialed iOS app (together, the “Service”) - operated by Koru Industries Inc. (“Koru,” “we,” “us”). By creating an account or using the Service, you agree to these Terms. If you are using the Service on behalf of an organization, you represent that you are authorized to bind that organization, and “you” includes it.
If your organization has a separate written agreement with Koru covering the Service, that agreement controls where it conflicts with these Terms.
The Service
Credentialed helps healthcare providers and credentialing teams store professional credentials, verify them against primary sources (such as state licensing boards and federal registries), monitor expirations and exclusion lists, and manage related documents and enrollment workflows.
Eligibility and accounts
- You must be at least 18 years old and use the Service for professional purposes.
- You are responsible for your account credentials and for all activity under your account. Notify us promptly at support@credentialed.health if you suspect unauthorized access.
- Information you submit - including credential details you attest to - must be accurate and yours to provide. Submitting another person’s credentials without authorization is prohibited.
Verification services
Primary-source verification results reflect what the issuing authority’s records showed at the time of the check. They are provided for informational and compliance-support purposes only:
- Verification results are not legal advice and are not a certification, endorsement, or guarantee of any provider’s qualifications or standing.
- Issuing authorities’ records can lag, change, or contain errors; a result can become outdated at any time after it is captured.
- Credentialing, hiring, privileging, and enrollment decisions remain solely the responsibility of the organization or individual making them.
Acceptable use
You agree not to:
- Use the Service to violate any law or the rights of others, including submitting false or misleading credential information;
- Probe, disrupt, or overload the Service, or attempt to access data or accounts that are not yours;
- Scrape, resell, or redistribute the Service or verification results as a competing offering;
- Upload malicious code or content you do not have the right to share.
Your content
You retain ownership of the information and documents you submit. You grant Koru the license needed to host, process, display, and transmit that content to operate the Service - including sending identifying details to issuing authorities when you or your organization request a verification. Our handling of personal information is described in the Privacy Policy.
Fees and subscriptions
Organizational use of the Service may be subject to fees under a separate agreement or order form. Features available free of charge today may change or become paid features in the future; we will provide reasonable notice of any such change that affects you.
Individual provider subscription (iOS)
- Independent provider workspaces include a 14-day free trial with full functionality, starting when the workspace is created. No payment is required for the trial.
- After the trial, continued use of paid features - running verifications and sanctions checks, uploading documents, and editing records - requires the Provider plan, currently $9.99 per month. Without a subscription, your existing records remain viewable and deletable, and credential-packet exports remain available but carry a watermark.
- Subscriptions are purchased through your Apple ID and billed by Apple. The subscription renews automatically each month unless you cancel at least 24 hours before the end of the current billing period. You can manage or cancel it at any time in your Apple ID subscription settings; cancellation takes effect at the end of the paid period.
- Refunds for App Store purchases are handled by Apple under Apple’s policies. If the subscription price changes, we will notify you in advance as the App Store requires, and the change will take effect only as permitted by Apple’s rules.
Termination
You may stop using the Service and delete your account at any time (in the iOS app under Settings, or by contacting support). We may suspend or terminate access that violates these Terms or presents a security risk. Sections of these Terms that by their nature should survive termination (including disclaimers, limitations of liability, and provisions about retained compliance records) survive.
Disclaimers
The Service is provided “as is” and “as available.” To the fullest extent permitted by law, Koru disclaims all warranties, express or implied, including merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the Service will be uninterrupted, error-free, or that verification results will be complete, current, or accurate.
Limitation of liability
To the fullest extent permitted by law: (a) Koru will not be liable for indirect, incidental, special, consequential, or punitive damages, or for lost profits, revenue, or data; and (b) Koru’s total liability for all claims arising out of or relating to the Service will not exceed the greater of the amounts you paid Koru for the Service in the twelve months before the claim arose or one hundred U.S. dollars ($100).
Indemnification
If you use the Service on behalf of an organization, that organization will defend and indemnify Koru against third-party claims arising from its use of the Service in violation of these Terms or applicable law.
Changes to these Terms
We may update these Terms from time to time. Material changes will be posted on this page with an updated “Last updated” date, and continued use of the Service after changes take effect constitutes acceptance.
Governing law
These Terms are governed by the laws of the State of New Jersey, without regard to its conflict-of-laws rules. Courts located in New Jersey will have exclusive jurisdiction over disputes arising from these Terms or the Service, and each party consents to venue there.
